Application for Criminal Record Status Certificate

1. ELIGIBILITY

 Those who are currently registered as (or whose last registration was as) a resident of Fukuoka Prefecture,
those who have alien registration status in Fukuoka Prefecture, and those who have been required by a
foreign public agency to provide certification of criminal record status may apply.
 Application must be made in person.  (Application by proxy will not be acceptable.)

2. OFFICE HOURS

 Weekdays (Monday-Friday)  9:30 am - 5:45 pm
 (Closed on Saturday, Sunday, national holidays.  Please enquire for end of year office hours.)

3. REQUIRED DOCUMENTS

□ For those who are residing in Japan

○ Japanese Nationals

  1. Proof of criminal record status certificate requirement such as the following:
    * Document issued by an embassy
    * Completed visa application
    * Document providing objective proof of necessity
    For details, please contact this office. 
  2. Passport 
  3. Document bearing the applicant’s name and address registered on the Resident Basic Register, such as his/
    her JUMIN-HYO [Resident Registration Certificate] (Issued by City Hall within the last 6 months),
     Driver’s License, or Personal Number Card
    * A notification was sent to you by the Ministry of Internal Affairs and Communications to let you know
    your Personal Number (or what is called your "MY NUMBER") and to request you to mail your
    application for your Personal Number Card (or what is called your "MY NUMBER CARD") to
    Personal Number Card Application Center's Po.Box.
      Probably, you have already received such a notification, and completed mailing your application; thereby,
    at present, some of you have received your Personal Number Card (or what is called your "MY NUMBER
     CARD").
      Regarding your Personal Number Card (or what is called your "MY NUMBER CARD") that contains
    your present address as your registered address on your Resident Register, the card is acceptable as an
    official document showing your registered official address when you apply for your Criminal Status
    Certificate at this office.
      Apart from whether you have already received your Personal Number Card (or what is called your "MY
    NUMBER CARD"), you are supposed to have the stub of your Personal Number Card application.
      Such a stub is titled as "Notification Card" (or "通知カード" in Japanese); this stub, however, is not valid
    for your Criminal Status Certificate application.

○ Foreign Nationals

  1. Proof of criminal record status certificate requirement such as the following:
    * Document issued by an embassy
    * Completed visa application
    * Document providing objective proof of necessity
    For details, please contact this office. 
  2. Passport 
  3. Document bearing the applicant’s name and address registered on the GAIKOKUJIN-TOROKU-
    GENPYO [Original/Master Document/Copy of Alien Registration], such as his/her
    TOROKUGENPYO KISAIJIKO SHOMEISHO [A certified photocopy of the original/masterdocument/
    copy of applicant’s Alien Registration held/issued by City Hall] (Issued within the last6 months), Driver’s
    License, Alien Registration Certificate, or Personal Number Card
    * A notification was sent to you by the Ministry of Internal Affairs and Communications to let you know
    your Personal Number (or what is called your "MY NUMBER") and to request you to mail your
    application for your Personal Number Card (or what is called your "MY NUMBER CARD") to
    Personal Number Card Application Center's Po.Box.

  2. Passport 
  3. JUMINHYO-NO-JOHYO [Proof of withdrawal from Resident Registration] or KOSEKI-NO-FUHYO
    [A certified photocopy of applicant’s family Permanent Domicile Registration including details of previous
    and current address/domicile] 
  4. Proof of current address (Foreign Driver’s License, Official mail containing your current address, etc.)

○ Foreign Nationals

  1. Proof of criminal record status certificate requirement such as the following:
    * Document issued by an embassy
    * Completed visa application
    * Document providing objective proof of necessity
    For details, please contact this office. 
  2. Passport 
  3. HEISA-SARETEIRU TOROKUGENPYO KISAIJIKO SHOMEISHO [A certified photocopy of the
    original/master document/copy of applicant’s Past/Previous Alien Registration held/issued by City Hall] 
  4. Proof of current address (Foreign Driver’s License, Official mail containing your current address, etc.)


IMPORTANT

  1. In a case  that you plan to bring your Resident Registration Certificate, please get such a printout on which
    your Personal Number (or what is called your "MY NUMBER") is NOT printed, and with it visit the
    Identification Division located  in the Police HQ;there, submit it to the officer in charge together with other
    required documents and forms with their blanks filled in when applying for your Criminal Status
    Certificate.
    If you have already obtained one with your Personal Number (or what is called your "MY NUMBER")
    printed, cross out your Personal Number (or what is called your "MY NUMBER") completely in a way
    such that any part of the number cannot be seen or read before submitting it.
     
  2. The documents produced by law firms and private agencies (such as mediators, promoters, or the like) are
    not acceptable as “proof of criminal record status certificate requirement”. 
     
  3. Please contact this office by telephone prior to applying/appearing at this office.  Depending on the country/
    region where applicant’s criminal record status is required and/or (on) the reason for applying for
    applicant’s criminal record status, his/her application may not be acceptable.

4. FEE

□¥400/one copy

5. ISSUANCE

□ Certification will be issued approximately 10 days after application.
 (Please allow 2 weeks  for end of year and Golden Week [or the holiday-studded week between the end of
April and the beginning of May in Japan].
□  Certificates may be collected in person or by a person who has power of attorney [in writing].
 (Certificates cannot be posted/maled.)

6. OTHER REMARKS

Residents abroad can apply through the Embassy of Japan.  However, if you are in the process of
applying through an embassy, you may not apply in Japan at the same time.
 Regarding how to apply outside Japan, please contact the Embassy of Japan where you will be making
an application.
